This form is to be completed by all volunteer candidates for any position with Victory Kidz. This form must be completed by the candidate and evaluated by Victory Church before the candidate will be included in ministry volunteer positions in Victory Kidz. This form is being used to help Victory Church provide a safe and secure environment for those who participate in our children’s ministry programs and use our children’s wing. The church will respect the private matters of the information collected.
