I. PARENT STATEMENT OF COOPERATION
A. We understand that enrollment in Foundation Christian Academy is not a right, but a privilege, and do affirm that there are standards that must be maintained for the welfare of each student, as well as, for the entire school. Therefore, we permit our students, teachers, and/or other school personnel to make and enforce school regulations in a manner consistent with Christian principles of discipline as outlined in Scripture and the Parent/Student Handbook. We will continue to uphold the authority of the teachers and staff of FCA by recognizing their right to use appropriate disciplinary measures. B. I have read FCA's discipline procedures and expectations. I agree to encourage my child enrolling to comply with the rules and conduct expected at all times – including evenings, weekends, vacations, and holidays. Furthermore, I agree to provide a home environment that supports FCA's expectations. C. I understand and agree to the school's right to provide consequences – including suspensions or separation from the school – to my child for violations of FCA's discipline procedures and expectations, which occur at any time while my child is enrolled at FCA, including violations that take place on evenings, weekends, vacations, and holidays. D. I agree to provide FCA with any information regarding arrests or convictions about my child while enrolled and understand that this information may be used by FCA to determine consequences. II. PARENT STATEMENT OF FINANCIAL RESPONSIBILITIES A. The operating budget of FCA is funded by tuition, fees, and contributions. Capital improvements are funded principally by gifts. Parents are asked to consider making both annual and capital gifts as an expression of their interest in and commitment to FCA. B. Parents are expected to pay their bills on time. Students whose accounts are 60 days past due will be restricted from attending classes, participating in extracurricular events, graduation exercises, and/or receiving report cards or transfer of records until the account is brought current. C. In event of academic failure, disciplinary reason, expulsion, or an extenuating circumstance all tuition monies, deposits, and fees already paid will be forfeited. Parents/Guardians have entered into a contractual relationship with FCA for the payment of ALL tuition and related fees for the entire school year. D. I understand that my obligation to pay the fees for the full academic year is unconditional and that upon receipt of this contract by FCA no portion of fees paid (i.e. registration fees, book fees, TMS fees) or outstanding will be refunded or canceled in the event of absence, withdrawal or dismissal from GRACE Association Schools of the above student. The only exceptions to this policy are 1.) In the event of a geographic relocation of the family beyond a 60-mile radius of Bloomington-Normal, IL. a prorated refund will be given, 2.) If a student withdraws before the first day of school, only paid tuition will be refunded. All fees are non-refundable. III. PARENT STATEMENT OF AGREEMENT A. I (we) hereby authorize FCA, and its acting leaders to teach and lead my (our) child in religious lessons and services, which may include prayer and Bible teaching. B. We agree to uphold the dress code as stated in the Student Handbook. We recognize the determination of whether our child complies with the dress code is solely at the discretion of FCA and we agree to have a cooperative spirit if any dispute arises concerning our child and uniforms. C. We have read, understand, and commit to abide by the policies outlined in the Parent-Student handbook. IV. PARENT STATEMENT OF INVOLVEMENT A. I commit to keeping my Parent-Volunteer hours with a cheerful attitude: 7 hours for part-time preschool parents, 15 hours per child with a maximum of 30 hours per K-HS family, or pay the appropriate amount in place of volunteering ($15 per hour). B. Actively participate in events sponsored by GRACE. C. Fully participate in fundraisers and meet fundraising commitments. D. Both the father and mother (where there is no separation) are expected to participate in their child's education. Both parents are expected to participate in teacher conferences, Parent-Teacher Ministry meetings, and Student Orientation Night. E. Respond to communications from FCA promptly. F. Read the Weekly newsletter distributed on Fridays to stay in touch with FCA and be aware of school activities. G. In keeping with the Biblical principle of offering grievances, (Mt 18:15-17), FCA strongly urges parents to refrain from gossiping; as such conversations are destructive to the administration, students, and the parents. Parents are expected to take the following steps to resolve a problem: 1. go directly to the appropriate teacher and not to other parents if there is a problem in your child's class, 2. go directly to the Principal and not to other parents if a problem arises that the teacher cannot handle, happens outside the classroom, or if you believe the situation is not being resolved, 3. go directly to the Administrator and not to other parents. V. PARENT STATEMENT OF MEDICAL RELEASE A. In consideration of GRACE Association Schools, allowing my student to participate regularly in field trips, sports, service projects, advisory activities, and extra-curricular activities for the current school year, I assume financial responsibility for all medical expenses should an accident occur. We understand that these field trips, sports, and activities include certain risks of accidental injury. We acknowledge that we have medical and/or hospitalization coverage and that we will look to that coverage for reimbursement. We specifically agree not to assert a claim or file a lawsuit against FCA or GRACE for personal injury damages should there be an accident or injury. If those circumstances occur, I agree to confine or limit any attempt to recover personal injury damages against the insurance carriers who provide liability coverage to FCA, to us, or third parties and/or their insurance carriers not affiliated with or connected to GRACE., who otherwise might be responsible for accidental injury. B. I (we) hereby permit my (our) child to attend and participate in activities sponsored by FCA and Student Ministries of Victory Church. C. I (we) hereby authorize any adult, in whose care the minor has been entrusted, to consent to any x-ray examination, anesthetic, medical, surgical, or dental diagnosis treatment, and hospital care to be rendered to the minor under the general or special supervision and on the advice of any physician or dentist licensed under the provisions of the medical practice act on the medical staff of a licensed hospital, whether such diagnosis or treatment is rendered at the office of said physician or at the said hospital. D. I (we) hereby authorize any licensed physician or medical treatment center to treat my (our) child in case of an emergency in which the before named physician cannot respond. E. Should it be necessary for my (our) child to return home due to medical reasons or otherwise, the undersigned shall assume all transportation costs. F. I (we) hereby release, forever discharge and agree to hold harmless FCA and the directors thereof from any liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses of any nature whatsoever which may be incurred by the undersigned adult the child-participant that occur while said child is participating in any trip or activity with FCA. G. The undersigned further hereby agrees to hold harmless and indemnify said school, its directors, employees, volunteers, and agents for any liability sustained by the said school as the result of the negligent, willful, or intentional acts of said participant, including expenses incurred attendant thereto. H. The medical consent and liability waiver provisions hereof shall remain in full force throughout the student's enrollment at FCA and in effect until written notice of revocation or withdrawal is received by Foundation Christian Academy at its main office at 18180 US Highway 150 Bloomington, IL 61705 It is the responsibility of the parent or guardian to notify the school of any changes in medical condition, guardianship, address, or phone change in writing to the address listed at the beginning of this form. My signature below affirms that I have read, understand, and accept the terms and conditions of this contract.
